Go to the nearest smart Civil Status registry kiosk. Select 'Birth Certificate', enter your data and the owner's National ID, confirm relationship, and scan your finger/face for biometrics. After confirmation, pay the fee displayed on the kiosk at the time of the request, and the kiosk will print your computerized certificate immediately.

Traditional Retrieval via Civil Registry (Cheapest option)

Visit the nearest Civil Registry office. Buy the retrieval form and pay the announced fee at the time of application at the cashier. Submit the form, your ID, and the owner's details to the officer, who will verify the record in the central database, print, and hand you the certificate.
